Compensation: $200 per completed admission visit + mileage reimbursement

Key Responsibilities:

• Conduct Start-of-Care (SOC) OASIS visits and create initial care plans
  
• Assess physical, environmental, and psychosocial needs
  
• Educate patients and families on home care expectations and treatment
  
• Collaborate with interdisciplinary teams for a seamless care transition
  
• Ensure timely and accurate documentation
  

What You Bring:

• Current RN license in Minnesota and CPR certification
  
• Previous experience in home health and OASIS documentation
  
• Reliable transportation and a valid driver’s license
  
• Ability to work independently and communicate effectively
